Community Care is always striving to simplify the claim submission process and maximize efficiency. We are currently working on implementing upgrades that will help to prevent claim submission errors including submission of duplicate claims and submitting with incorrect billing information.

Please note that the following bill types have been removed: 867,868,327,328. Also, we would like to remind you that CCI has created a TOB (Type of Bill) chart that is available on the bottom of the CSP homepage if there are any questions what Type of Bill you should use to submit your claim.

Failure to submit the correct type of bill may result in claim denial. Community Care Inc. is required to report a valid type of bill to the Centers of Medicare & Medicaid Services (CMS) and the State of Wisconsin Department of Health and Human Services (DHS).
